Description: At 6:02 p.m. on Friday, August 11, 1995, a southbound Yonge-University-Spadina line subway train ran into the rear of a parked train halfway between St. Clair West and Dupont subway Station. There were about 200 to 300 passengers on the two trains. Of those, 3 women were killed and more than 30 people were taken to hospital with injuries. After 40 years of subway operation with a perfect safety record, it was the first time that people died on the system and it was the TTC's fault. What caused the accident? 1. Robert Jeffrey, the train driver, was only on the job for two days; he consecutively ran red lights. 2. A trip arm, which was supposed to activate the train's emergency brakes at red lights, had a design flaw. TTC: The Safer Way? Prior to the 1995 crash, it faced: -A subway system that was in such advanced disrepair that long segments of track could have been condemned. -A signal system that malfunctioned with astonishing frequency and jaded train drivers who would routinely run red lights to save time. -A geriatric bus fleet in the midst of mechanical and structural collapse. -An old-boy management clique plagued by self-satisfied incompetence. -A corporate structure that discouraged communication between key departments, including the maintenance personnel who detected problems and the engineering division that fixed them. -A hubristic focus on grand subway expansion at a time when the existing system was aging and wearing out. Thankgoodness he had no serious injuries but it was a long road to recovery for both of us. I was a Jr. member of a SAR group, we assisted with getting supplies to the firefighters and paramedics below ground.
